Creating Value by Bringing Hospital Care Home

The American Hospital Association along with other partners are employing a model that enables qualified patients to receive hospital-level care from home. This shift assigns a health provider who oversees the at home care through remote monitoring and other interventions without the burden of an in hospital stay. This model reduces costs as well as increases outcomes.
